Attention community members:
If this is the first time logging in to this website, or if you have already logged in but have forgotten your password, you will need to set a new password for your account.
To do so please follow these steps:
- Click on the tab (above) marked "Request new password" and enter your email address. Alternatively, you can type in your "username" which has been configured as your "Firstname Lastname". DO NOT ENTER A PASSWORD.
- You will receive an email to your email address with a special, temporary link back to the site that will permit you to reset your password.
- Create your password. Note that you are logged in at this point.
- Now review and update your profile and patient experience to keep it up-to-date.
