We're now receiving nominations for the Patients' Choice Awards in Sarnia and we need your help to judge the winners! We'll meet and review nominations at the Culture of Cities Centre located at 561 Bloor St. West on the 3rd floor (closest intersection is Bathurst/Bloor).
I'll send everyone who confirms an electronic version of the nominations closer to the date. If you're unable to attend you can still participate by emailing your winners before September 13th.
